It's not AI, at least not how we normally envision genAI. He basically made a way to rig DAZ3D directly to his own server, so it auto-creates and edits the models there based on JSON configs and then send them back to your computer. In theory this allows effectively infinite variations and more gradual growth in each body part without increasing the tech debt or performance requirements on your own machine. As well as possibly generate full CGs dynamically.
This is still an insane thing to do for a game without a central gameplay loop or a complete idea. And the fact he is still tied to DAZ3D models, it still doesn't look good to either making his own Blender shapes or just a standard paper doll! This isn't an MMO, there is no reason to make the game like this!

Famished is interesting cause it seems like the author has a decent idea where he wants to take the game, a good foundation on how to mess around with genAI art (still need to work on the consistency and make sure he uses a proper VAI, without one that's how you get those washed-out CGs), and despite the wide scope it all seems doable. It also helps it goes all in on the Vore and does not pretend to have any forms of moral. All is food to the stupid dog girl.
I'm still somewhat skeptical if it can keep up the consistent quality. It definitely needs something of a combat update and art update and probably tone down its rising ambition. But I've had a good time with it.

That kind of walk-around-a-sandbox-and-fatten-up-everyone game isn't that notable a concept. Feeder Fantasy was just good because of the cute art and writing, not really a gameplay concept.
